Lipotherme FAQs

What is Lipotherme?
Lipotherme is a safe, effective procedure that uses a laser (similar to those used in other surgical procedures) to liquefy and remove unwanted fat in targeted body zones. In addition to removing unwanted fat, Lipotherme’s thermal effect causes the skin to contract, resulting in firmer, smoother skin that further enhances your body shaping goals.

The clinical term for this type of procedure is laser-assisted lipolysis (also called laser lipolysis or laser lipo for short) which is an evolution of traditional liposuction.

How does Lipotherme work?
Dr Porcaro, Board Certified by The American Board of Cosmetic Surgery*, will first administer a mild numbing agent to the targeted body zone. Then a small metal tube called a cannula is inserted under the skin and a tiny laser beam is fired. Dr Porcaro moves the cannula back and forth. As the laser warms your fat cells, it creates a thermal effect.

Fat cells, which are highly receptive to thermal energy, are liquefied and removed permanently. As an added benefit of Lipotherme, the heat produced during the treatment tightens your skin, creating a more balanced skin texture for you.

Is Lipotherme the same thing as liposuction?
Lipotherme and liposuction are both highly effective procedures for the removal of unwanted fat. However, Lipotherme laser lipolysis is a minimally-invasive procedure with virtually no downtime.

Traditional liposuction is considered a more invasive procedure and may require several days of down time. Lipotherme is excellent for treating certain areas that traditional liposuction doesn’t treat easily, such as the bra line, under the chin, above the knees and ankles.

Depending on your goals and physical condition, Lipotherme can be an alternative to traditional liposuction, or Dr Porcaro may recommend a combination of Lipotherme with traditional liposculpture (laser liposuction Port St Lucie).

How soon will I see results and how long will my results last?
Typically, you’ll see dramatic improvement within 1 to 2 weeks after the treatment, but some areas such as the neck and jaw can show even faster results. Because healing and metabolism rates are different with everyone, your results following a Lipotherme treatment usually require 12 to 16 weeks to become fully apparent and improvement continues to be seen for several months.

Is this a “one-time” treatment?
Lipotherme treatments are considered “one-time” treatments. In adults, fat cells don’t grow back once they have been removed. If you gain weight after having Lipotherme, the excess fat will accumulate in areas where you did not have the treatment.

Is there a recuperation period?
Typically, you can resume many activities the day after your treatment. It is recommended you wait several days before resuming strenuous activity such as exercising, however.

Is Lipotherme safe?
Yes! Lipotherme laser-assisted lipolysis has been approved by the Food and Drug Administration (FDA) for use in the United States.

Is there anyone who is not a good candidate for Lipotherme?
Yes. Laser Lipotherme, like liposuction and liposculpture, is not a weight reduction procedure and is best suited for patients with isolated pockets of fat that are resistant to diet and exercise. Dr Porcaro, Board certified by the American Board of Cosmetic Surgery*, sees his best results in patients who are at or near their ideal weight. Patients with BMI’s between 30-35 are “borderline” candidates, while morbidly obese patients with BMI’s of 40 or more are not candidates for this procedure.

Women who are pregnant should not undergo Lipotherme.

Tumescent Liposculpture FAQ

How much fat can be removed
Up to 4 liters in one session irregardless of what venue the procedure is performed. This is the same amount as with traditional liposuction.

If I gain weight following Tumescent Liposculpture will I gain it in the area treated?
No. The fat cells are permanently removed during tumescent liposculpture, so your treated area can never become as large as it would have been if the procedure had not been performed.

How safe is Tumescent Liposculpture?
Tumescent Liposculpture at Porcaro Cosmetic Surgery Port St Lucie is the safest method of liposuction available. Large studies involving over 15,000 patients reported no hospital admissions for complications, and there were no blood transfusions required. Reported complications were minor and infrequent.

Is Liposculpture painful?
There is minimal discomfort during the administration of the anesthetic. You will have the option to have oral or intravenous sedation before the procedure. What is the worst part of the Tumescent Liposculpture procedure?
The leakage of slightly blood-stained anesthetic fluid from the tiny 2-3 mm incisions, lasting for about 1-2 days after the procedure. The refinement of not closing these small incisions accelerates the healing process and reduces bruising.

What is required after my Tumescent Liposculpture?
A garment (like a girdle) will be worn for 2-4 weeks after the procedure. Proper diet, physical exercise and weight reduction (if necessary), are essential to maximize your outcome.

Who is suitable for Tumescent Liposculpture?
Although the ideal candidate is a person who is in good health, exercises regularly, maintains good eating habits and is within 10kg of his or her ideal body weight, the procedure can have a dramatic effect on moderately obese individuals as well. It is the most effective method for removing localized fat deposits in body areas which will not improve with diet and exercise. It is not a treatment for generalized obesity, however. The most common areas of localized body fat in women include the lower abdomen, upper inner and outer thighs, hips, waist, buttocks, inner knees, arms and neck. The common problem areas for men include the abdomen, waist and neck. Tumescent Liposculpture allows safe and effective correction of these problem areas in patients of almost any age, from 16 to 80 years.

What are the potential common side effects?
Bruising, swelling and temporary numbness are possible and irregularities in the skin can occur after tumescent liposculpture even though microcannulas are used. These conditions slowly resolve in 12 to 16 weeks with noticeable improvements in 2 to 4 weeks.

How much will it cost?
It depends on the area being treated. Tumescent Liposculpture at Porcaro Cosmetic Surgery Port St Lucie is safer and less expensive than liposuction performed under general anesthesia.

Why don’t all doctors use the tumescent technique if it truly is the safest and most precise?
Tumescent liposuction takes about twice as long to perform because the physician has to inject the anesthesia slowly so you remain comfortable and pain free. What type of anesthesia is used?

How is the procedure performed?
After instilling a local anesthetic into your areas to be treated, Dr. Porcaro will wait about ½ hour before starting the procedure. During this waiting period the areas will become numb. Liposuction may precede removal of excess skin and muscle tightening.

How painful is it?
There will be some pain requiring the use of a narcotic for the first several days.

What will I need to do after the procedure?
You will need to wear an abdominal binder & avoid anything strenuous for between 2 & 4 wks. Most patients will require a week off from work.

Buttock augmentation FAQ

What will the procedure do for me?
Reshaping and augmenting the buttocks has become a very popular body sculpting procedure. You may have adequate volume in your buttocks to start with and want additional enhancement while other women have little or no fat, leaving them with a more manly back side. You will enjoy your shapelier & sexier figure afforded by liposuction combined with fat transfer for buttock reshaping and enhancement.

Am I a candidate?
Buttock augmentation is best performed with your own fat, recovered at the time of liposuction. You need to have a sufficient amount of fat that can be harvested, however, as we need a minimum of 800-1000 ml fat per buttock in order to produce a meaningful change. If you have insufficient fat stores, you may need to gain weight!

What about buttock implants?
Buttock implants have fallen out of favor because their use carries a significant risk for complications. It is preferable to reshape and augment the buttocks with your own fat.

Is it safe & where is the procedure performed?
Your procedure will take place safely & comfortably as an outpatient in Dr. Porcaro’s AAAHC accredited facility. There’s a small risk of bleeding, especially if you have not stopped Aspirin & NSAID’S before the procedure.

There is a charge for the liposuction component, depending on the number of areas you want to have treated as well as a separate charge for the fat transfer.

What do I do before the procedure?
You will need to be off blood thinners and ensure that your blood pressure is well controlled. You may require medical clearance from your family doctor depending on your age & your health. You will need to make arrangements to be off from work and to be driven to the office for your follow up appointments for 3 weeks after your procedure.

What type of anesthesia is used?
You will appreciate the IV sedation you receive from Dr. Porcaro’s RN during the procedure. Local anesthetic will also be utilized, which minimizes discomfort and blood loss and facilitates liposuction, harvesting of your fat and fat transfer.

How is the procedure performed?
After adequate IV sedation, the areas to be treated will be numbed with a local anesthetic. Liposuction of the abdomen, flanks, back, inner and outer thighs and possibly the arms and other areas will be performed to reshape these areas and to harvest fat. Once your fat is harvested it will be separated from the local anesthetic, concentrated and & then strategically placed into the buttocks for ideal contouring and enhancement.

How painful is it?
This procedure will require the use of a narcotic for several days after the procedure.

What will I need to do after the procedure & what should I expect long term?
You will need to avoid sitting, lying on your back and anything strenuous for 3 weeks after the procedure. The final result may take several months. Some patients will require another fat transfer session to achieve the shape and volume they desire, as only about 60% of the fat that is transferred will survive.

When will I see Dr. Porcaro again?
You will see Dr. Porcaro several days after the procedure.

Sclerotherapy FAQ

What will it do for me?
Injecting spider & small varicose veins of the legs will improve the appearance of your unsightly veins, give you more confidence & enable you to be more comfortable wearing all types of clothing.

Will my insurance cover it?
No. It is regarded as a cosmetic procedure when it is not associated with problems with larger veins.

Is it safe?
Yes! Dr. Porcaro will use Asclera to treat your veins. This FDA approved product is very safe and has quickly become the preferred vein injection agent.

How much does it cost?
The change for a 20-30 minute session is $350. We offer a discount when more than one session is performed on the same day. You may require multiple sessions, depending on your goals & the magnitude of your problem.

Does it hurt?
Asclera has anesthetic properties so there’s minimal to no pain or discomfort with the injection.

When will I see the effect and how long does it last?
It may take up to several weeks for the treated veins to disappear. Once the veins disappear they won’t return. However, you will probably get more veins that will require treatment in the future.

What are the risks?There’s a small risk of swelling or bruising at the injection sites, no different than any other injection. There’s a remote chance you can develop blood clots in your leg.

Who does the injections & how are they performed?
Dr. Porcaro will perform a Doppler exam & determine if you are a good candidate for sclerotherapy. Your skin will then be prepped with alcohol and the spiders & small veins will be injected with Asclera that is mixed with a local anesthetic. Dr. Porcaro uses a fiber optic light- “The Vein Light”- to ensure the greatest accuracy with each individual injection, performed while you are lying down.

What limitations are there afterwards?
There are no special precautions after undergoing sclerotherapy; you can continue with your daily routine. Support stockings are strongly recommended after the treatment.

Will I see Dr. Porcaro again?
You will return for a complementary follow up appointment 2 wks after the treatment. By this time the maximum effect of your treatment will have taken place. Other veins can be injected at that time.

What’s a labiaplasty as opposed to a vaginoplasty?
Enlarged labia minora may be congenital or it may develop after pregnancy. Loose vaginal tone is typically the result of vaginal births. Labiaplasty removes excess tissue from the labia minora (outer vaginal lips) while vaginoplasty removes excess tissue & tightens the vagina & surrounding muscles.

What will labiaplsty do for me?

Hand Rejuvenation FAQ

What will it do for me?
Next to your face, your hands are the most conspicuous part of your body! If the appearance of your hands is making you look or feel old, self conscious or out of sync with your improved appearance after other cosmetic treatments, hand rejuvenation is for you. Having younger and more attractive hands may be as simple as a filler injection, vein injections and or medical grade skin care.

What injectable products are used?
Fillers such as Juvederm, Radiesse, Sculptra or your own fat can be use to restore lost volume in your hands.

How are the injections performed?
Dr Porcaro will cleanse your skin and administer a local anesthetic. He will then inject the filler you have chosen, either Juvederm, Radiesse, Sculptra or your fat thru a single site, using a small cannula. This flexible, blunt microtube allows for even distribution of product. The cannula skirts around your veins, minimizing discomfort and the chance of black and blue. Cannulas are safer and more comfortable than sharp needle injections.

What about my prominent hand veins?
Prominent hand veins often become less noticeable after adequate volume restoration with fillers. When veins remain unsightly after volume restoration, they can be injected with Asclera (sclerotherapy) or removed thru tiny incisions.

What about my skin?
Thin, rough, wrinkled skin with or without age spots may respond to simple volume restoration with fillers. Other measures, such as medical grade skin care products, chemical peel, laser or light therapy may be needed.

When will I see the changes and how long will it last?
Volume restoration with fillers may show immediate or delayed improvement, depending on the filler you select. Fillers may last between 6-12 months and will require regular maintenance. Medical grade skin care products, including the use of sun block, require ongoing maintenance.

How much does it cost?
Depending on your particular situation, fillers may cost $1500 to $2,000. Using your own fat to replace lost volume with cost $3,000. Vein injections cost around $400. Skin care products will cost several hundred dollars. Chemical peels are in the range of $500. Laser & light therapies will cost over $1,000

What are the risks?
Risks are low with hand rejuvenation. Swelling on the tops of your hands can last from 1-10 days. Black and blue and or a bluish cast may be apparent for 1-2 weeks. Temporary itching, pain and redness are also possible.

What are the limitations afterwards?
You will need to elevate your hands, apply ice and may also be instructed to massage your hands. Your hands will peel after a chemical peel, laser or light therapy treatments so a bandage may be needed. Strenuous manual tasks are discouraged for the first week after filler injections.

Depending on your treatment, you will be seen by Dr Porcaro in 2-4 weeks.

What type of anesthesia is used?
IV sedation is used along with numbing medicine instilled into the areas being treated, which numbs the nerves, loosens the fat & constricts blood vessels, making this procedure well tolerated and safer than general anesthesia.

How is the procedure performed?
After adequate sedation, Dr. Porcaro will make a 1 in. incision below your chin and incisions in front of (for men) and within the ear (for women), curving behind the ear a variable distance. Liposuction of the neck and jowls may be performed at the same time. The supporting structures of your neck & lower face will be tightened & your excess skin will be removed. Your incisions will be closed with absorbable suture material that will not need to be removed.

How painful is it?
There will be some pain requiring the use of a narcotic for the first several days.

What are the risks?
There’s a risk of bleeding, especially if you have not stopped Aspirin & NSAID’S or if your blood pressure is not adequately controlled. There’s a very small risk of weakness in the lower lip which typically resolves on its own.

What will I need to do after the procedure?
You will need to apply ice & take it easy, keep your head elevated & avoid anything strenuous for about 7 days. You will wear a Velcro strap 24 hrs/day for the first week & then at night for the next week.

When will I see Dr. Porcaro again?
On the day after the procedure, your dressing will be removed and a Velcro strap will be applied.

Blepharoplasty FAQ

What will it do for me?
The removal of excess skin and or fat around the eyes will give you a more youthful, refreshed look, often times taking 10 years off your appearance!

Will my insurance pay for it?
Unless you have documented vision loss because of sagging upper eyelid skin that interferes with your sight, Medicare and other payers will not cover this procedure. Insurances almost never cover the treatment for bulges of fat or loose lower eyelid skin.

How much does it cost & where is it performed?
We offer competitive pricing when compared with similarly trained physicians in our area. The charge depends on whether you require treatment of just the upper or lower eyelid or if you need both upper & lower eyelids treated. If you require correction of loose lower eyelids, additional charges will apply. You will receive a discount when both upper & lower eyelids are treated at the same time and when eyelid surgery is combined with lower face & neck lift surgery. Your procedure will take place safely & comfortably in Dr. Porcaro’s AAAHC accredited facility, adjoining the office.

What are the risks?
Although the risk is small, the biggest threat is bleeding, which can occur during or after the procedure. Your risk of bleeding is low if you don’t have elevated blood pressure and are not taking any blood thinners like Aspirin & other NSAID’S.

What do I do before the procedure?
You will undergo an eye exam with Dr. Porcaro to assess your vision & tear production. You may need medical clearance from your family doctor depending on your age & your health.

What type of anesthesia is used?
You will receive oral or IV sedation along with a local anesthetic.

How is the procedure performed?
Once you are comfortable & relaxed, the areas will be cleansed & the excess skin and fat will be removed through an incision in your natural skin crease so it won’t be noticeable. A suture will be placed below your skin in the upper eyelid and a butterfly strip placed over the suture. Often, no sutures are needed when bulging fat is removed from your lower eyelids.

How painful is it and what can I expect afterwards?
You will experience minimal discomfort for the first day or two. You may experience swelling & trace black and blue that may take a week to resolve.

What will I need to do after the procedure?

Like fat, Sculptra is injected deeply, just above facial bones and is not the preferred product for superficial lines & wrinkles, which are best addressed with Botox and or Juvederm.

How are the injections performed?

How painful is it?

You will enjoy the IV sedation you will receive from Dr. Porcaro’s RN during this procedure. Local anesthetic minimizes discomfort and blood loss and facilitates liposuction.

How is the procedure performed?
After adequate IV sedation the areas will be numbed with a local anesthetic. Excess fat in the breasts will be removed with liposuction. After removing fat, most patients will require excision of the breast gland tissue itself because the glandular material is too hard to be suctioned. This will require a small incision thru the nipple or along the lower aspect of the areola. Absorbable sutures are used to close your skin so sutures will not need to be removed.

How painful is it?
This procedures is typically not very painful although there will be some pain requiring the use of a narcotic for the first day or two days.

What will I need to do after the procedure?
You will need to apply ice & take it easy and avoid anything strenuous for several weeks.

When will I see Dr. Porcaro again?
You will see Dr. Porcaro several days after the procedure.

What will it do for me?
Hair restoration performed by Dr. Porcaro will result in a significant improvement in your appearance. Dr. Porcaro and his team transplant individual hair follicles as they exist naturally for truly natural results. In addition to improving your appearance, you will feel better about yourself and be more confident.

Is it safe?
Yes; very safe! Your procedure will take place safely & comfortably in Dr. Porcaro’s AAAHC accredited facility, adjoining the office.

How much does it cost?
The price is dependent on the amount of hair being transplanted. We offer discounts on larger sessions. We have very competitive pricing when compared with similarly trained physicians in our area.

What do I do before the procedure?
You will need to be off blood thinners and ensure that your blood pressure is well controlled. You may require medical clearance from your family doctor depending on your age & your health.

How painful is it?
You will experience some discomfort that will be relieved by a pain pill; Tylenol or Motrin may be sufficient

What are the risks?
Providing you are in good health, there is virtually no risk with a hair transplant procedure.

What type of anesthesia is used?
You will receive local anesthesia and you will have the option to take Valium for relaxation before or during your procedure.

How is the procedure performed?
After numbing the areas, Dr. Porcaro will remove the donor tissue and create the micro-incisions that will dictate how the new hair will grow: the angle, spacing, density & direction of the hair. At the same time, Dr. Porcaro’s team will dissect the hair into individual follicles, count the number of follicles & transplant the follicles into the micro-incisions Dr. Porcaro created.

What will I need to do after the procedure?
You will need to avoid any strenuous activities for about a week & keep the transplanted grafts moist so they don’t dry out. The biggest challenge is having the patience to wait for your new hair to grow! This typically occurs about 3 mos. after the procedure & may take a full year to get your final result.

When will I see Dr. Porcaro again?
You may return the day after your transplant if you live in the area. If you are from out of the area you will have access to Dr. Porcaro by cell & email. Dr. Porcaro will be available to assist and guide you through out your recovery.

Age Management FAQ’s

How often will I need blood work?
Blood work is necessary before starting on Bio-Identical hormone replacement. After your first pellet insertion, you will need to repeat the same blood work in 3-4 weeks. Coupled with improvement in your symptoms, this will tell Dr. Porcaro weather your initial dose was just right, too much or not enough. If your blood work is within the target range, you are doing well and getting pellet inserts regularly, you may not need to have blood work repeated for 12 months.
At Porcaro Cosmetic Surgery, we won’t ask you for blood work unless we really need it, saving you time, the inconvenience & discomfort as well as the expense!

How will it help me?
Having optimal hormone levels will have a profoundly beneficial effect on your post menopausal symptoms such as night sweats & hot flashes. Your immune system, your sex drive, emotional stability, and cholesterol modulation will be improved. With Bio-identical hormones, you can virtually prevent breast cancer, Alzheimer?s disease and memory loss, osteoporosis, heart disease, stroke and diabetes! How is it done?
Dr. Porcaro will inject local anesthesia into the area & introduce your hormone pellets after making a small nick in the skin. A steri- strip is applied over the insertion site; no stitches are used.

Do I need to sign a contract or make monthly payments for pellet hormones?
No! Unlike most facilities that offer pellet hormones, we have no contract agreement what so ever! At Porcaro Cosmetic Surgery you pay for what you need when you need it!

How long will I need pellets?
Unfortunately, once a hormone deficiency has been documented there is no expectation for improvement on its own, so you will need life long treatment in order to maintain optimal results.

What about Progesterone?
Progesterone works in conjunction with estrogen to relieve menopausal symptoms, protect against breast & uterine cancer, prevent osteroporosis, improve heart health & enhance overall feelings of well being.
Natural progesterone, when used with estrogen, provides women significant relief of many symptoms of aging and menopause and is indicated in ALL postmenopausal women irregardless of weather on not the uterus has been surgically removed. In addition to the health benefits of progesterone, there are ‘feel good benefits’ as well! Progesterone helps women sleep so we recommend that women take progesterone just before getting to sleep.

When will I see Dr. Porcaro again?
At the time of your next insertion-typically 4 mos. for women & 5 mos. for men

What about Estrogen?
Estrogen protects against heart disease and stroke, decreases cholesterol, prevents and lowers the incidence of Alzheimer’s disease, improves memory, decreases symptoms of menopause, decreases insulin resistence and prevents osteoporosis

What about Testosterone?
Testosterone can produce improvement in well being, energy, body composition, bone density, sexual function & clitoral sensitivity. Testosterone protects against deterioration of muscles, ligaments, joints, tendons, bone and skin and also helps to prevent skin wrinkles and skin thinning from loss of collagen.
Even though testosterone is the principal male sex hormone, women need testosterone their entire lives too.Testosterone enables a women to live a vibrant life as she ages.

Do I need it?
Most women & men over the age of 40 and ALL women who have gone thru menopause will have insufficient hormone levels to function optimally. Treating hormone deficiencies documented by blood work with natural, Bio-identical hormones is the safest and best form of hormone replacement for you.

What about over-the-counter products containing phytoestrogens, and topical products containing low-dose progesterone?

Are there restrictions afterwards?
You will need to leave the water proof tape on the site for 6 days. You can shower but you need to avoid baths, hot tubs, pools, etc for one week.

How often will I need pellets?
Pellets are long acting and typically last women 4 mos. and men 5 mos.

Does it hurt?
You may experience minimal discomfort at the injection site for the first several days. It is unlikely that you would need to take a pain pill.

Do you accept my insurance & will my insurance cover it?
We do not accept insurance or Medicare. We will provide you with coding information that you can submit to your insurance carrier. We have seen some patients get full reimbursement while other patients have gotten no reimbursement.

Is it safe?
Dr. Porcaro replaces deficient hormones with bio-identical estradiol & testosterone in pellet form, which are smaller than the size of a tic tac. These hormones are grown in soy & are chemically indistinguishable from estradiol & testosterone you already have in your body.

What about The Women’s Health Initiative (WHI) study?
There has been controversy on the topic of Hormone Replacement Therapy (HRT) for the last 15 years. Negative attitudes about hormone replacement have dominated the media since 2002. Combine this with the Women’s Health Initiative (WHI) study that was halted three years ago because of health risks, women and physicians are having diffulty making informed decisions on their health.

Let?s shed a little light on the confusion. First, the WHI study and the subsequent media reports about HRT are based on Commercial-synthetic- HRT only- not natural hormones! They are based on findings from use of the combination of conjugated estrogens and medroxyprogesterone (called PremPro). Thus, when you hear about HRT in the news, this is what they are referring to. The complications seen with these synthetic products are NOT seen with Natural- Bio-Identical- hormones! These Natural Hormones are SAFE!
