Financial PolicyPorcaro Hair & Cosmetic SurgeryCONSULTATION: There is no charge for cosmetic surgery or hair restoration consultations. Bio-Equivalent Hormone Replacement consultation charge is $175.00. Payment for injectable products (i.e. Botox, Juvederm, etc.), minor surgical procedures, aesthetic services and products for sale are due the day the services are rendered. FEES: The fee includes surgeons fee,anesthesia fee, and facility charge for your specific procedure.The fee does NOT include post-op medications,garments,lymphatic drainage, x-rays, laboratory, pathology, or radiology charges, nor does it include emergency room visits, hospitalization, office visits or procedures with other physicians or FUTURE SURGERY by Dr. Porcaro.
PAYMENT OPTIONS: Cash, cashier’s checks and money orders (made out to Porcaro Cosmetic Surgery), major credit cards (Visa, Master Card, American Express and Discover) and approved financing (CareCredit, Surgery Loans & Chase Healthcare Advance) are accepted methods of payment. PERSONAL CHECKS ARE NOT ACCEPTED. Approved methods of payment in place of cash are payment in full for goods and services rendered. There is no further recourse for these charges. Your signature is in good faith for all treatments and products.
DEPOSIT: A non-refundable deposit of $1000.00 is required in order to schedule a surgical procedure. Your surgery must be paid in full 7 days before the procedure. All deposits are NON REFUNDABLE CANCELLATION POLICY: We understand that a situation may arise that could force you to postpone your surgery. Please understand that such changes affect not only your surgeon but other patients as well. Dr. Porcaro’s time, as well as that of the operating room staff, is a precious commodity and we request understanding.If we are notified 7 days prior to surgery we will reschedule your surgical appointment. If you need to cancel a surgical procedure with less than 7 days notice, your deposit cannot be returned and the deposit cannot be used for a future procedure. If you choose to reschedule the procedure, you will be asked for another non-refundable deposit of $1000.00 to secure your space on the surgical schedule REVISION POLICY: For any revision or “touch up” procedure there is a $500.00 minimum charge to schedule. If a revision is needed after 1 year from procedure date there is a $1000.00 minimum charge to schedule. INSURANCE: We are not affiliated with any insurance companies or plans and do not accept insurance payments for services.
