Fees, Scheduling & FinancingFee schedule effective Nov 2011*
Prices are for cash payments. The use of approved credit or debit cards (i.e.: Visa, Master Card, American Express and Discover) will result in a 3% transaction fee. If you elect to finance through one of our financial partners, there will be a 6 % transaction fee. Above fees include the surgeon's fee, facility fee, anesthesia fee and all follow up with Dr. Porcaro. *An additional charge will be applied for patients undergoing a surgical procedure where IV sedation is utilized whose BMI exceeds 30. We do NOT participate with Medicare or any insurance. We can provide you with billing codes so that you may submit to your insurance company for possible reimbursement. The patient and any other person responsible for payment has a right to refuse to pay, cancel payment, or be reimbursed for payment for any other service, examination, or treatment that is performed as a result of and within 72 hours of responding to the advertisement for the free, discounted fee, or reduced fee service, examination, or treatment. Scheduling/Payment: Scheduling a date for your surgical procedure requires a $1,000 nonrefundable deposit. Payment of the balance is required at least 7 days before surgery. Payment for injectables (i.e. Botox and fillers) is required on the day of the procedure. Cash, checks, cashier's checks, money orders and most major credit cards are accepted. Financing: Financing options, independent of this office, are based on your credit report findings and are subject to review before approval.
